Help! Why’s my crockpot beef stew still tough after hours in the crockpot?

Hi, everyone! I've had my beef stew in the crockpot for ages, but it's still coming out tough. I thought slow cookers were supposed to make everything super tender! Am I missing something? Any tips or tricks would be hugely appreciated.
