Why do my pork chops end up tough when cooked in a crockpot?

Hi friends! I am in the middle of crockpot recipe in the kitchen and i was disappointed the pork chops are hard and tough.. anyone has ideas why? Will try it this weekend, any secrets on making those chops very tender and juicy?
